Smith County, TX — A Texas Department of Public Safety crash report lists “fatigued or asleep” as a contributing factor in a July crash that killed 34-year-old Chelsea Breanne Dillard.

The crash happened around 6:18 a.m. July 23 on FM 14, north of Tyler Loop 323.

According to the DPS report, the Uber driver was traveling north in a 2025 Nissan Sentra when the vehicle crossed the double yellow line, entered the southbound lane, left the roadway and struck a fence.

Dillard, who was a passenger in the vehicle, died from injuries sustained in the crash.

The report also states that Dillard was not wearing a seatbelt.

Uber described the crash as a tragedy and said it was prepared to support law enforcement as the investigation continued.
